"Tolerable exploitation". I agree with you on this point, of course. A middle man used to be called "Mr/Mrs ten percent" and played a valuable part as intermediary in business. The tax-payer funded Councillor, however, providing you with garbage collection, pothole filling, car parking etc. should not be amongst the 'exploiters' demanding 'bonuses' for doing the job they are already well paid for.

I see in today's British papers that one Council head on an annual salary of BP160,000 (more than the Prime Minister), hasn't attended her office since November. We are not allowed to know why she can't work for her salary and in my opinion this is not 'tolerable exploitation' but outright abuse of public office.

It is high time for me to introduce the concept of "tolerable exploitation" to anyone who thinks non-profit =/= COMMUNISM.

COMMUNISM is a FAILURE because no one wants to be FORCED to give their stuff away for free. Not labor, not land, nor family jewels. Oh sure, people get a benevolent streak going here and there, and we can be grateful for that. But in general, people need POSITIVE MOTIVATION. Not a bayonet in their back...

Thus "tolerable exploitation" is a reasonable rate of return for investors, and that comes from profits of an enterprise, the remainder going to current leadership, be they executive managers or founders. Growth (equity) = profit, so reinvestment beyond basic maintenance is ALSO profit! Managed growth, rate of return tied to interest rates, and the FED out of interest rate fixing is all needed. I would expect investor returns of 7% would work out great for everyone, as would interest rates. If reinvestment and entrepreneurial rates were similar, you get managed growth, stimulus to create jobs, and wages tied at least partially to productivity rather than stagnating at 0% for 40 years against inflation (at least in the USA).

A national sales tax of 7%, and some stable and expected currency devaluation on a schedule can fund normal governmental functions. The US Dollar continues as reserve currency, and a resurgence in the US economy will keep it there.

The ENTIRE WORLD must start to pay taxes to the US in the form of mild currency devaluation over time out in the open. As bad as US interventionism & war profiteering in the Middle East really is, they amount to not much more than a "hobby war" for Zionists that sit atop the thrown of Amerisrael. Stop and think! Imagine the world WITHOUT the USA. It would be run by Russian Oligarchs or perhaps the Chinese....

I shan't snag you at all. I agree totally. Although I think medicine, education and facilities should be Non Profit. By which I mean no margin between gross and net after all R&D etc have been catered for. I disagree totally with the idea that to get the right man for the job necessitates searching within an extremely small pool of connected genes and coming up with a 'mate' who's reward for success/failure is equally rewarded and way outside the norm.

I used the 100mill figure because both major hospitals in my hometown are CONSTANTLY expanding, and are constantly looking for financing. Interesting that you use the word 'reserves'. I would think that 'reserves' would be a function of 'profits', the theme of our discussion. I do think that something should be done about the obscene profits that the drug manufacturers make. And I don't entirely believe their claim that "We have to make uber profits to finance R&D." I'm okay with 'profits', but not to the point where they hold people 'hostage'. You'll probably snag me on this one.

Yes of course I'd be looking at the figures especially their reserves. Suddenly finding themselves in need of $100 ml investment for improvements suggests shortsightedness on behalf of the management. I'd expect it to be a 'gilt edge' company paying fixed interest/dividend.

I could split hairs and say that improvements come under expenses but never mind. Let's change it to hospital extension for the sake of your argument.

It's taking me a while to grasp the depth of your determination to be drop-dead resolute. I won't try to amuse you again. I get the impression that you have had some 1st hand experience with morbid healthcare situations. I will, however, regale you with some of my daughter's medical experiences. One such being the treatment of a young lady who, traveling at night along a rural Florida highway, feeling the urge to relieve herself, pulled off the highway, walked into the palmetto, and squatted on a rattlesnake. Sara's 1st snakebite patient. Large snake. Lots of venom injected. Did you know that anti-venom is 10K$ per vial? I was flabbergasted when Sara told me that.----- Well anyway, the patient survived, (with the requisite nasty scar) and a 90K$ hosp bill. Hey wait, YOU made ME laugh.----- when you said "and the single payer is um-wah" That was a funny, right? (Maybe you ain't so drop-dead resolute after all, darlin')

I apologize if 'troll' offended U. I have redacted all of my comments regarding use of 'troll' on your behalf. Any investment carries a modicum of risk. Ordinarily, greater risk means potential greater reward. The inverse is obviously also true. But you haven't answered my ? about a potential investor looking at a hospital's bottom line. Let's try role reversal. Let's say a hospital needs 100 mill in capital improvements. (Not hard to imagine) Hosp Administrators are looking for investors. You are a potential investor. Would YOU be looking @ balance sheet?

Then good. What right have charities, hospitals and clinics got to think in such terms? Profit. Profit. Profit. Growth. Bullshit. Let's have a little value for money for a change. Let's have a bit of philosophical thought on the subject of where it's all heading. You all defend the CEO's paycheck of $25million per annum as though he had actually sat down 9 - 5pm and earned (can't be arsed to do the math) per hour. Come on. The bubble burst for most everyone and yet you argue to keep the 1% fully pumped up in theirs as though the rest of us will perish without their crumbs. We'll see a bit more 'trickle down' when theirs is burst and the benefits are more equitably spread.

As I argued elsewhere, I support the sort of model that John Spaden Lewis advocated when he set up John Lewis in Britain: "INDUSTRIAL DEMOCRACY" he called it, and looking at the balance sheets of such companies and the 'happiness' level of their staff/shareholders/customers, I shall plump for this model. It would seem to represent a win-win situation, so rare nowadays.

There are trees in the grounds of the Government Hospital, Delhi. It is shady and the sound of birds gives a tranquil feel to the place. A woman is slumped on the roots of one of the the shady trees... stone dead. The A and E department is below ground, down a concrete ramp. Exactly like an underground multi-storey car park. As you descend, you begin to see blood on the walls and the sounds of excruciating pain. An old man sits on an old charpoy by the wall, his stump of a leg wrapped in gauze and dripping blood. He has received no pain releif and groans while he rocks back and forth. At the bottom of the ramp, where there would normally be parked cars, hundreds of charpoys are lined up in the gloom, each with the filthiest sheet you ever saw. Every patient is an accident or emergency. In the actual 'consulting room' 4 young Indian doctors beaver with little or no equipment on patients lying in blood and gore.

It is obligatory (or it was) for Indian doctors to serve their first year in such conditions and I expect they learn a great deal from the experience and take my hat off to them.

However, that is India.

Personally, I would go for dropping the "stellar" salaries (why would anyone need $23 million per annum to live on?). Why should I pay ten times the price of a generic aspirin just because a copy of it is in a bit of tinfoil with a fancy name on the box?

I see so many ways to bring down costs before reducing services and yet you won't even countenance such a solution.

offense = "BagTroll". Troll is not what I am.

Investors should not be 'gambling' on a health system. That suggests a volatile market. Wherever the money is raised to provide this 'human right' should be fixed and safe. I'm afraid I am pro the British NHS even though successive governments, both left and right, have set out to destroy it. It is (as I speak) being carved up and handed in neat parcels to 'mates' and to date, the result is proving disastrous. (similarly in education)

Arguing that Revenue - Expenses = Value is not as rewarding as Revenue - Expenses - Profit = Value flies in the face of logic.
