Ayatollah Ali Khamenei is not merely a clerical strongman; he is the living embodiment of a theocratic delusion that has wreaked havoc across the Middle East for more than four decades. As Supreme Leader of the Islamic Republic of Iran — the ultimate authority over the state, the military and the Revolutionary Guard — he blends absolute religious conviction with ruthless political power, creating a toxic worldview that is both messianic and merciless.
At the core of Khamenei’s psyche is an obsession with divine mission. As leader of Iran’s Shiite Twelver sect — a branch of Islam that venerates the succession of twelve imams as God’s appointed guides — Khamenei perpetuates the notion that he is not just a political ruler but a spiritual commander. In his own propaganda he draws parallels between himself and Imam Ali, the first Shia imam, implying that opposition to his rule is tantamount to opposition to God’s will. This kind of self-deification is typical of ideologues who interpret worldly power as sacred duty, and it isolates him from empathy, rational compromise, and democratic legitimacy.
Psychopaths in power often describe their aims in cosmic terms: mission, destiny, purification. Khamenei fits this psychological profile. He views dissent not merely as political disagreement but as heresy that must be stamped out. Under his command and ideological justification, Iran’s security apparatus — especially the Islamic Revolutionary Guard Corps and Basij militias — has become an instrument of terror against civilian life.
The regime’s response to the 2025–2026 nationwide protests — sparked by economic collapse and political desperation — provides chilling evidence of this mindset. After peaceful demonstrations erupted, security forces carried out what independent observers and human rights groups have described as massacres on a scale unprecedented in modern Iranian history. Tens of thousands of civilians were shot, beaten or executed during crackdowns, particularly around January 8–9, 2026, when death toll estimates range from thousands to more than thirty-six thousand killed by live fire and other means.
These figures do not represent battlefield casualties — they represent unarmed men, women and children crushed under the heel of a regime that would rather drown its own streets in blood than relinquish theocratic dominion. The systematic nature of these killings — and the insistence by state media to label demonstrators as “rioters” — reveals a complete absence of empathy, compassion or moral restraint.
Khamenei’s ideology is inseparable from the rigid dogma of Shiite clerical rule. Shiite Twelver theology, as manipulated by Iran’s ruling elite, positions the clergy as ultimate interpreters of divine will. In practice, this worldview has justified discrimination against women, religious minorities, and anyone deemed insufficiently devout. Women in Iran have endured systemic oppression — from compulsory dress codes to exclusion from public and political life — all enforced under clerical edict. This is not benign religious culture; it is institutionalized misogyny sanctified by law. Non-Muslims and even fellow Muslims of different sects are treated with equal contempt. The regime classifies other faiths and interpretations as apostasy or threat, often sanctioned by state violence or legal discrimination.
This theological hierarchy — where only the clerically authorized interpretation of Shiism is permitted — breeds intolerance and justifies persecution. The contempt is structural, not accidental: it flows from a worldview that values obedience over freedom, uniformity over individuality, and coercion over consent.
Psychopathy in leadership reveals itself in one unmistakable marker: a lack of empathy. Khamenei’s Iran has repeatedly demonstrated this in its treatment of dissent, its refusal to acknowledge the humanity of its victims, and its systematic suppression of truth. The internet blackouts imposed to conceal evidence of the massacres are not defensive measures; they are tools of state terror. For a leader who styles himself as a shepherd of the faithful, the reality is starkly different: he is a tyrant who sees citizens not as human beings with intrinsic worth but as obstacles to be eliminated. This is the hallmark not of rational governance, but of dangerous delusion married to brutal power.
The United States and its allies cannot continue to engage with this regime as if it were a responsible state actor. The psychological profile of Khamenei reveals a leader driven by divine mission, sectarian supremacy, and an utter disregard for human life — a combination that poses a direct threat to regional stability and global security. Iran’s current internal crisis — with protests spreading internationally and record numbers of civilian deaths — is not merely a moment of upheaval, it is a crisis of legitimacy for a regime that has abandoned any claim to moral authority.
As democratic nations confront this reality, the West must prepare for the hard truth: dealing with a theocratic psychopath at the helm requires a strategy rooted in both moral clarity and decisive action.
The Iranian people deserve freedom, dignity and self-determination — not a madman’s dream of divine rule imposed at the tip of a bullet.

With respect, Khameini is no more a “madman” than was Muhammad. The source of the “psychopathology”, if that’s what it is, is the Quran. It is shared by anyone who is devoutly Muslim.
And yes, it is still true that “If you have one psychopath with a thousand followers, you are, in effect, dealing with one thousand and one psychopaths”.
As JohnXY points out in his comment preceding mine, Rehov fails to understand that what he believes is madness and psychopathy is merely adherence to mainstream, orthodox Islam. Khameini does nothing not in accordance with Islam; Khameini knows his Islam thoroughly, and his actions and mindset are, by Islamic standards and precepts, pious and unimpeachable. Rehov (and countless others, including Trump) evidently remains clueless about this grim reality.

Islam’s’ 1400 year history is largely a record of conquest and subjugation. By one estimate, approximately 170 million people have been murdered in the name of Allah, making Islam the greatest killing force in history by far.
This is pure. unadulterated arrogance combined with pure, unadulterated power. Fifty thousand murdered demonstrators is the result of this unholy deviance led by, in his own mind, the holiest of all.
Khomeini better have his escape planned out. Otherwise the people will be dragging him and his regime through the streets when they get overthrown…..sometime next week.
If Khameini is best described a psychopath or a sociopath , in either case he is dangerous malicious and outright vicious . Iran does not need a miniaturized light weight Nuke to put on top of a sophisticated ICBM delivery system and our military leaders know that. A crude and heavy but effective Nuke like “The Gadget” during the Trinity test put on a ocean tug boat, container ship, even a large sailboat and it floats into NY harbor, LA , Boston etc… and half the city is gone in an instant. Iran is NOT going to be allowed to develop a nuke. Nope. They either completely give it up, dismantle it and agree to random spot inspections to verify……..or they will not know what hit them and we will set their military capability back so far they will be riding camels again.

Ayatollah Khamenei is very much viciously against the Iranian people, For he and the other ayatollahs mullahs and similar Muslim despots in tyrannical control in that Islamic regime of Iran terribly oppress the Iranian people through their thugs in uniform who are called the “Islamic Revolutionary guards.”
All those years of miserly oppression suffering and murder imposed on the Iranian people by those despots of that Islamic tyranny of Iran is coming back down on those evil dictators of that horrific regime. Therefore, the old saying might a valid statement, which is “What goes around comes around.”
For the Iranian people are right and justified for hating that brutal cruel oppressive and even murderous Islamic tyrannical regime of Iran.
For example, the many arrest and imprisonment and even execution of people , repeatedly, who had peacefully protested that “mullah tyranny” of Iran only further exposes the weak ,fearful and intolerant nature of those in power in the oppressive and brutal “mullah regime “ of Iran.
Therefore, the Iranians people are more than justified in wanting that brutal cruel oppressive Islamic tyranny to be overthrown and replaced with a genuine government.
As a former Muslim revealed an important reality when he wrote, “The Islamic republic of Iran exists and operates as what every fundamentalist dreams of, an Islamic state ruled by Sharia …” He further exposes that “What followed its establishment was the inevitable consequence and inexorable logic of its Islamic premise; state terrorism, a merciless tyranny.” [1]
